About This Lesson
Please complete this certificate and return a copy to the AFT/Share My Lesson’s approved CTLE Sponsor along with your PD certificate. You must retain your copies for eight years. It is not necessary to send a copy of this form to the Office of Teaching Initiatives unless it is requested in the event of an audit or for use in obtaining an Initial Reissuance. A separate form must be completed for each training.

Additional Notes: As of 2016, the State Education Department requires Continuing Teacher and Leader Education (CTLE) for all holders of professional classroom teaching, educational leadership, or Level III teaching assistant certificates. Details on exact rules and how to track your hours can be found on the SED website.
Share My Lesson is now an NYSED-approved provider for CTLE requirements.
Step 1: Upon completion of the Share My Lesson webinar, download your PD certificate of completion.
Step 2: Complete CTLE certificate.
Step 3: Send both your CTLE certificate AND Share My Lesson PD certificate of completion to affiliates@sharemylesson.com.
Step 4: Maintain your personal records of completed continuing teacher and education leader (CTLE) as directed by NYSED here: https://www.highered.nysed.gov/tcert/resteachers/ctle-record-doc.html
